Voluntary Benefit Options

Voluntary Accident Insurance

This plan pays towards injury-related ER visits, hospitalizations, follow-up visits, physical therapy, and coverage for off-the-job accidents.

Provides a $50 annual wellness benefit for completing certain routine wellness screenings or procedures for each covered member.

Pays injury benefits from $25 to $25,000

Includes accidental death benefit of up to $25,000.

Voluntary Hospital Indemnity Insurance

Benefits are paid directly to you when you need it most and can be used however you choose to help pay for out-of-pocket medical expenses like co-pays and deductibles or for non-medical expenses.

Covers sickness and injury

Provides a $1,500 hospital admission benefit (to a max of 1 admission per year, per insured)

Daily hospital confinement benefit of $200 (to a max of 15 days per year, per insured)

Voluntary Critical Illness Insurance

Benefits are paid directly to you as cash benefits that correspond with a variety of covered illness, such as: cancer, vascular conditions, neurological conditions, comas, and more.

Choose: Benefits to protect yourself and any covered family members, if diagnosed with a covered critical illness.

Use: If you or another covered member are diagnosed with a covered illness.

Claim: Go online and file a claim. The cash benefits are paid to you, to use however you wish.
