Life insurance can provide income protection for you and your family. Some coverage is provided automatically to you at no cost. Other supplemental coverage is available to purchase based on your needs.

Life Insurance

Basic Life and Accidental Death & Dismemberment (AD&D) Insurance

Capstone Nutrition provides hourly employees with $25,000 of Life and AD&D and provides salaried employees with up to 1x your annual salary to a maximum of $200,000 as part of your core benefits. This coverage is completely free to employees — Capstone Nutrition pays the premiums. Additionally, you have the option to convert your coverage if you retire, lose eligibility or terminate your employment with Capstone Nutrition. This benefit is provided through Reliance Standard.

Voluntary Life Insurance

Capstone Nutrition offers Voluntary Life for you and your dependents, which can be purchased through Reliance Standard. This option gives you the advantage of purchasing life insurance and AD&D at affordable group rates. It is not a pre-tax benefit option. Premiums are paid with after-tax dollars through an automatic payroll deduction from your paycheck. These coverages are completely voluntary.

You may purchase additional coverage in increments of $10,000, not to exceed five times your annual salary or $500,000. If you enroll when you are first eligible, $200,000 is available without proof of good health. Benefits reduce beginning
at age 65.

Voluntary Dependent Life Insurance

You may purchase spouse coverage in increments of $5,000, not to exceed $250,000. If you enroll when you are first eligible, $50,000 is available without proof of good health. Benefits reduce beginning at age 65.

Children’s insurance coverage is for unmarried dependent children from age 14 days to age 20 (age 26 if unmarried, full-time student). You may purchase up to $10,000 of children’s coverage, in increments of $2,000. Infants age 14 days to 6 months have a flat benefit of $1,000. Coverage is inclusive for all children. This means that if you have one child or many children, you pay one flat amount; however, each child is covered individually for the coverage amount.

Around-the-clock travel assistance.

Whether you need help with an injury or illness, lost passport or luggage, or even a prescription refill, you can rest easy knowing you have access to a personal travel emergency companion.

How your Travel Assistance services work:
Using your travel emergency services is a cinch! Just contact On Call International directly at (603) 328-1966 anytime you need assistance while traveling. On Call’s Global Response Center is open 24 hours a day, 365 days a year and can provide the following services through your coverage with Reliance Standard. When traveling more than 100 miles from home or in a foreign country, On Call offers you and your dependents the following services:

  • Passport/visa requirements
  • Currency exchange rates
  • Consulate/embassy referral
  • Health hazard advisory
  • Weather information
Emergency medical transportation services*
  • Emergency evacuation

  • Return of traveling companion, dependent children, vehicle, and/or mortal remains
Emergency personal assistance services
  • Urgent message relay
  • Interpretation/translation services

* The services listed above are subject to a maximum combined single limit of $250,000. Return of vehicle is subject to a $2,500 maximum.
