Voluntary Short-term Disability

Benefits Begin: There is a waiting period before benefits are payable. Benefits begin on the 15th day of injury or illness.

Weekly Benefit: 60% of weekly earnings, not to exceed the plan’s maximum weekly benefit amount less other income sources.

Maximum Benefit Period: Benefits are available for up to 11 weeks.

Maximum Weekly Benefit: $1,500

This benefit is provided through LifeMap and Venafi pays 100% of the premiums for this coverage.

Long-term Disability

Benefits Begin: There is a waiting period before benefits are payable. Benefits begin on the 91st day of disability.

Monthly Benefit: 60% of monthly earnings, not to exceed the plan’s maximum monthly benefit amount, less other income sources.

Maximum Benefit Period: Social Security Normal Retirement Age

Maximum Monthly Benefit: $8,000

Pre-existing Conditions: Disabilities that begin within the first 12 months after your effective date will not be covered if you have received treatment for the disability within the 3 months prior to your effective date.

This benefit is provided through LifeMap and Venafi pays 100% of the premiums for this coverage.
